Document Template is the template type that you will be using for this review.  This is a choice field.  The options that are listed have been set up by the Master Advisor in Templates.  A template must be selected to move on to the next screen.

Review Date is the date that this review is set for.  This date is picked up in the portfolio information.  To change this date, you can either enter the date in the format day/month/year, or you can use the calendar selection option by clicking the down arrow.

Once you have entered this information, click the Next button at the bottom of the screen to continue.  To exit this screen, without saving the information, click the Cancel button at the bottom of the screen.
