Group Name this is the name of the group that you are creating.

Advisor - Select the advisor who owns the group. This field will only be enabled if you have more that one advisor in your practice.

Notes are personal comments made by you about this group.

Once you have entered this information, click the Next button at the bottom of the screen to continue.  To exit this screen, without saving the information, click the Cancel button at the bottom of the screen.
