This screen is the default screen when:

It is here that you create the questions for the questionnaire, specify the scoring for each question and test the questionnaire that you have created.

The information on this screen is discussed below:

At the top left hand side of the tab folder the template name is shown in bold letters, below this is the description of this template.  Next to this on the right hand side, is the option to Edit Template.  This option allows you to edit the template name and the template description.  When you select the Next option the Sections Edit screen is show so that you can make changes to the sections and their weightings.

Below the Edit Template option is a button with the option to Lock this Template.  The risk profile templates can only be used once they have been locked.  A template must only be locked when you are sure that it is complete.  No changes can be made once the template has been locked.

Add Question is where the questions for the questionnaire are entered into the template.

Test allows you to test the questionnaire that you have created and evaluate the scoring.

On the left hand side of each question are up and down arrows.  These allow you to change the sequence of the questions on the questionnaire.  Use the up arrow to move a question up by one place or the down arrow to move a question down by one place.

On the right hand side of each question is a Delete option.  Use this option to remove a question completely.

Once you have entered this information, click the Save button at the bottom of the screen to continue.  To exit this screen, without saving the information, click the Cancel button at the bottom of the screen.
